2. Booth Type:
Plaza level is where the stage is located.  Street Level is the landscaped areas along Smith Street.

__________ Plaza Level half booth 10' across x 5' deep: $85. 

3. Number of Booths:

4. Table Rental:
 Would you like to rent an 8-foot, rectangular table (the festival pays to rent these and have them delivered and picked up)

Would you like to purchase additional admission tickets at $12 each? If so, how many? (Note booth rental includes 2 vendor passes. Food vendors get 4 passes.)

6. Required: Provide a brief description of your booth.  Applications cannot be considered without a description.  New vendors, provide a web link or photos if possible.  For vendors that are selected, we will post this description on our web site and publish it in our event guide.
